I've had myself and two children insured with IHI for 4 years (they cover two children for free per each adult member), and have found them to be excellent. They were the only company willing to insure me including my pre-existing condition (for an additional % penalty on the premium, I am covered for my condition).
The only hassle is that their card is pretty much not accepted over the counter anywhere I go here in HK as an outpatient, so I find I do have to keep a spreadsheet running to track our outstanding claims, usually takes about 3 weeks for auto-reimburse to our account. Occasionally I have had to "remind" them re certain claims that have become stuck in the system, but the response via e-mail is within hours and always positive.
For large hospital stays which are planned, pre-authorisation can arranged within 24 hours via a special e-mail "hotline" for that purpose only. Works beautifully.
We have found the investment in our medical care and peace of mind to be well worth the cost (especially over a particularly trying pregnancy). I'm sorry, but personally, wrestling with the public system for myself and my children is something that I am overjoyed I can afford not to do (and the same goes for my home country).
