i found DR CORA NGAI who works at the queen mary, to be very open minded and EXCELLENT. she delivered my triplets and was supportive, caring etc the entire time.
however, the doctor is bound by the rules of the hospital. the queen mary, for example, does not allow husbands in for emergency c-sections. a doctor cannot change that rule. i believe the queen mary also has rules regarding when epidurals can be used. (anaesthesiologists work only certain hours, or something like that, i could be mistaken). again, a doctor cannot change the rules.
cora ngai is an excellent doctor. however, i believe the doctors there work on a rota system, and there is no way of guaranteeing who delivers your baby if you go into natural labour.
