Help! Lost immunization card!

Hi! I have been taking my child to the govt MCHCs since birth but I recently lost my immunization card! Has anyone ever misplaced theirs? Do they have copies at the MCHCs? Ahhh!!!
 
I have the same problem! I called the MCHC and it seems they can issue a duplicate. They said you need to go there with the child's birth certificate and your ID card and fill in a form. It will take about 2 weeks to get the duplicate. I haven't actually been there to fill in the form yet but will do so in the next few days. Good luck!
 
I'm certain it can be replaced. They have all the records, too, and are probably prepared for such things to happen ;)
