Doctor's Certificate??

I have been asked to provide a Doctor's Certificate stating my current medical condition (i.e. pregnant, etc.), but dont know what that is. In the US, we usually get our doctor to write something on his prescription pad or write a letter - is this the same in HK or is there an actual Doctor's Certificate form that they fill out?

My doctor is a bit abrupt so I was hoping to have things prepared for her before I go see her next time.

Whether it's for your employer or anyone else, a letter written on headed paper will suffice.
If your doctor is a government doctor, there may be a standard form according to the purpose.
 
Thanks kashismum - do you know if a doctor's official stamp means anything, if no headed/letterhead paper is available?

The doctor's official stamp should be good enough. My gyna wrote the confirmation ofmy pregnancy on a blank paper and my employer accepted it.

I am unsure of your question as you seem to have just copied and pasted the original post... Is it to give to your OB or work? It is basically a piece of paper provided by a doctor to prove that you are pregnant. It will normally be on headed paper with the doctor's name and address and it.
 
I had to get one of those. It means that you need a letter from the OBGYN stating that you are pregnant, how far along you at that time the letter was written and when you're due.
 
it's normally a note on your doctor's letterhead and with your doctor's chop stating how many weeks you are and that you are fit to fly. if you are doing round trip, make sure you get one for the way back too (or the one note covers that period of time). when i flew to nyc for the summer, i got my singapore doctor to write one when i flew home, then my nyc ob wrote another one for my trip back....
