Birth Plan Template for Queen Mary

Does anyone know if there is a standard template for documenting your birth plan for Queen Mary/public hospitals in HK? All the birth plans I googled are pretty US-centric and have numerous sections and delve deep into areas which I am convinced are not offered at public hospital.

I don't really know where to start, and who I should give my birth plan to.
