Anyone given a birth plan to Union Hospital?

I have booked for a standard room at Union Hospital.

I am hoping to prepare a birth plan, but may i know who do you give the birth plan to sign and agree to it? My OBGYN is actually attached with Union Hospital, so do i give him my birth plan and also a copy to the hospital to sign-off in agreement? Do you write the birth plan in English or Chinese? I dont know chinese, but i can try and find someone to do the translation for me.

I am planning to include the following in my birth plan:
1) To be able to choose my birthing position - instead of the usual lying on the bed position;
2) To have skin-to-skin contact with my baby immediately after giving birth and to suckle my breast to stimulate breastmilk
3) No vaccinations to be given to my baby

What else did i miss out? Thanks all!
 
I gave birth in December at Union, you can give your doctor the birth plan and also a copy to the midwife nurse and maybe the admission nurse when you check in. English is okay as everyone speaks at least decent English.
I'm not sure if you can choose your birthing position, their beds are the standard pull up your knee kind...
#2 and 3 you can do for sure, just make it clear with your doctor what you want.
